By signing this card I authorize AFSCME New Jersey Local 2779, Child Care Workers Union (CCWU) to be my exclusive representative for purposes of collective bargaining and I agree to abide by its Constitution and Bylaws.

I am a public pay childcare provider

I hereby voluntarily authorize the deduction of dues from each subsidy check, regardless of whether I am or remain a member of the union, in the amount set by CCWU, and as they may be adjusted periodically by the union. I further authorize the State of NJ to remit this amount to CCWU Council 63.

This authorization shall be irrevocable for a period of one year, and for year to year thereafter, unless I give CCWU written notice to opt-out not less than 20 days before the end of any yearly period.

As a private pay childcare provider, I agree to pay the current monthly rate of $28 by the 15th of each month to: AFSCME New Jersey, CCWU Local 2779, 2653A Whitehorse–Hamilton Sq. Rd., Hamilton, NJ 08690.
